Status before joining IPC:
Hypertension, Hyperlipidemia Post PCA & post CABG status) Again advised for CABG. Bypass surgery (21/011993) /and angioplasty done earlier, Again started chest pain and advised for repeat cardiac surgery. Cartography shows reverse blood flow deficiency to heart (17/03/05).

Medical History Chest heaviness on brisk walking, after food on climbing upstairs. Shortness of breath with suffocation feeling on walking and on climbing stairs, tiredness. No chest heaviness Breathlessness reduced Walking everyday for 30 mins. Tiredness reduced very well.
Stress Test Positive. Moderate exercise tolerance. Improved effort tolerance
2D echo EF = 50% EF = 50%
Cartography Severe blood flow deficiency to heart. Normal blood flow to the heart.
Mental status Business related stress, short tempered. Negative thoughts comes in mind, worrying about health. No negative thoughts. Able to control anger.

Age / Sex 66 / Male
Occupation: Retired (Architect).
Address (Resi) : Wada-Manor Road, Kalam Khan, Wada, Thane
 
Status before joining IPC:
Angina pain, Angiography showing RCA = 90%, LCX = 80% & LAD = 90%, blocked and he was advised bypass surgery.

Improved pumping of the Heart (EF = 50%)
Stress Test:
ECG changes and chest pain while walking on TMT. (Positive TMT)

No ECG changes and no chest pain while walking on machine. (Negative TMT)
Angiography
Severe blockages of 80-90% in all major coronary arteries. Poor collateral circulation.

Total reversal of block in one artery & it is total clear & other blockages are reversed to 40-50% only.
Yoga & Meditation
He was not doing it.

Now he is doing it regularly for 30-45 mins.
Walking :
Not walking, unable to do so due to chest pain.

Regularly walking for one hour.

Diabetes mellitus. Suffered heart attack. Angiography showing :- Triple vessel disease with proximal LAD = 90%, LCX = 99% & RCA = 20% blocked & he was advised urgent bypass surgery.

ECG changes while walking on treadmill machine. (Positive Test)

No ECG changes & complaints while walking on treadmill. (Negative Test)
Coronary Angiography
Total 5 blockages with multiple critical blockages of 90-99% in major coronary arteries. Poor collateral circulation.

Reversal of 3 out of 5 blockages and remaining 2 are reduced in severity from 99% to 70%. Excellent development of collateral circulatory.
Mental status
Feels stressed out all the time

Feels relaxed with regular practice of yoga and meditation.
Walking :
Unable to walk due to chest pain.

Walking everyday for 45-60 min.

• Heart Attack twice , 1st in Aug. 02 &2nd in Nov. 04 • Angiography done in Nov. 04 showing Triple vessel disease • RCA – 90 %- 95 % LCX – 80 %, LAD – 90% blocked and he was advised coronary bypass surgery.

Status before joining IPC:
• Diabetes for 20 years • Ischemic heart disease for 3 months and angiography done on 19th Nov. 2003 revealed triple vessel disease (LAD = 70%, LCX = 90%, RCA = 75% blocked). He was advised Bypass Surgery.

Medical History
Chest pain, which used to increase after exertion, thereby limiting his office work.

Doing routine office work – 8-10 hrs. which includes traveling to Borivali Office & Bhyander factory.
Blood Sugar Values :
Fasting = 213, Post Lunch = 247 mg%

Better control of blood sugar
F = 147, PP = 164 had to reduce diabetic medicines.
Cartography Report :

Stress Test:
Stress test positive , ECG changes while walking on the treadmill.

Lack of confidence and fear about heart disease.
